USING ORDER SETS (User's Manual, pgs. 823-826)
Orders Sets can make your order entry process faster and less prone to errors by presenting you with lists of commonly used Orders. You can create as many different kinds of Orders Sets as you need for various purposes.First, let's create an Order Set:
1. Press View\Orders\Utility\Edit
Order Sets.
2. In the Order Set window, type in a name you want to use for the Order Set, which reminds you of its purpose. For this example, let's call it ARTHRITIS.
3. Now, let's add some tests to this Order Set. Press the Add button.
4. Type in ANA in the Test to Add box, then press Ok. Notice that ANA is now added to the Order Set Editor in the Orders list.
5. Continue to add tests by typing RHEUMATOID FACTOR, URIC ACID, and ESR and pressing Ok after each one.
6. Press Quit to close the Add Test to Order Set window.
7. Note that you can automatically alphabetize the list if you wish by pressing Sort; or you can move tests around within the list by pressing the Up or Down buttons or by simply dragging a test up or down on the list using your mouse. If the set is long, you will probably find it handy to have it alphabetized, but if it is short, it may be handier to have the more commonly ordered tests at the top of the list.
8. Note that you can also import the contents of one Order Set into another Order Set by pressing the Import button and then selecting the Order Set you want to copy into the Order Set you are currently editing.
9. When you are done editing the Order Set, press Ok to store it.
Now, let's use this Order Set for writing Orders on Kelly Erickson:
1. Press View\Orders\Utility\Use Order Sets.
2. Enter Kelly Erickson as the name of the patient.
3. In the Title box, start to type in ARTHRITIS, and choose it from the list.
4. Notice that the Order Set window is automatically filled in with the tests from the Order Set. Each is checked by default as being ordered. Each is also automatically assigned by default to the first Diagnosis on the list. The Diagnoses are automatically filled in from the patient's Diagnosis Editor.
5. Let's check RHEUMATOID FACTOR and SED RATE but uncheck ANA and URIC ACID.
6. However, we also want to add a C-reactive protein. So, on the 5th line, below SED RATE, start to type in C-REACTIVE PROTEIN, and then select it from the list of matching test names. Then assign it to diagnosis #1 by typing "1" in place of 0 or by clicking on the up arrow button next to "0" to increase it to "1". Note that you can keep adding tests to order, or even replace tests on the Order Set with other tests if you need to. You can also assign NO diagnoses to an order by having its number set to "0" or backspacing over the number to make the box blank. (However, for billing purposes, it is wise to have a relevant diagnosis assigned to each test).
7. Note that you can also order things besides tests (e.g. "PHYSICAL THERAPY"). You just need to add them to the Lab Book Vocabulary so that they are recognized. Simply type in a term in one of the Orders windows, and if it isn't recognized by the Vocabulary System, the Test Name Editor opens, and you can add it to the Vocabulary on the spot, and then use it for your Orders.
8. Move to the first Diagnosis window. Note that you can press the down arrow button to the right of each diagnosis box, which brings up a list of the patient's current diagnoses, from which you can pick. Or, you can simply start to type something into the box, and the program lists all of the diagnoses in the Vocabulary System that being with those letters. Use this to enter a diagnosis of RHEUMATOID ARTHRITIS for the diagnosis.
9. It doesn't matter what the other Diagnoses are on the list, since we are assigning all of the ordered tests to Diagnosis #1. However, if you wanted to assign some tests to one diagnosis, and other tests to another diagnosis, you can associate the tests with up to 8 different diagnoses. Note that you can jump to a diagnosis window by pressing <Alt> and the number (1 through 8) or by using <Tab> or using your mouse to click on a window.
10. Let's add a Comment that these tests are for follow-up. (Use of the Comment field is optional).
11. Note that you can press the Jump button to move to other portions of the patient's chart, if you need to view information there before deciding what you want to order.
12. Version 4.6 adds an Ordered by window which lets you specify which provider is ordering these tests.
13. When you are done ordering the tests, press Ok to store them.
