Patient Portal

Your patients can access portions of their medical records 24/7 over the Internet using the Patient Portal. The Patient Portal Permission Log lets you fine tune which patients are permitted access, and the CCD Log records each time a patient views information and what was displayed.



CCD Exporter

Export a patient's chart to a file in CCD (Continuty of Care Document) format in seconds using the CCD Exporter. CCD files can be read by any electronic health record program that complies with Meaningful Use, so you can easily share patient information with the patient's other physicians by sending CCD files.

CCD-CCR Reader

You can read CCD (Continuity of Care Document) and CCR (Continuity of Care Record) files using the CCD-CCR Reader. All electronic health record software compliant with Meaningful Use must be able to generate either CCD or CCR files. Thus, other physicians can send you patient data in this format and you can review it in Welford Chart Notes.
